New Patient Appointment Request

    You may fill out the form below to receive a call for a new patient appointment. All information submitted to us is kept strictly confidential. New patients can also schedule by calling 757-366-0692 and dialing "1" for appointments.

    Note:  All HMO insurance plans (including Tricare Prime) require you to notify them of who your primary care physician will be before you are seen. Please make sure this information is updated before scheduling.

We accept most of the plans offered by the following insurances:

Telephone

Our office staff is available by telephone during regular office hours. Our telephones are turned over from 12:30pm to 2:00pm, Monday -- Thursday for administrative work and lunch. Telephone options to cancel appointments and to address urgent care referrals are available 24 hours a day. 

After Hours

An on-call staff member is available after hours to authorize urgent care referrals by dialing our office phone number. However, all non-emergent matters, including refills, appointments and referrals, will need to be handled during regular office hours.


​Wheelchair

A wheelchair is available for the use of our patients. If needed, please call our office prior to your appointment.

​​Monday- Thursday: 8:30am to 5:00pm
Friday: 8:30am to 12:00pm 

Closed from 1:00pm - 2:00pm (lunch)

Appointments & Walk-ins

Appointments & walk-ins are offered during the following hours:

Monday -- Thursday: 8:30am-- 12:30pm, 2:00pm-- 4:30pm
Friday: 8:30am to 11:30am (office closes at 12 noon)
Walk-in sick visits requirements:
  • for existing patients with recent symptoms (ex. stomach pain, UTI, rashes, etc.)
  • in order of arrival​
Please note:
  • If you have COVID symptoms please call before walking in. 
  • Walk-ins will be seen by the first available healthcare provider. We will make every attempt to accommodate walk-ins. ​
  • For guaranteed appointments, please schedule in advance.
  • If walk-ins cannot be accepted on specific dates, we will update the message on our phone system. 



​Inclimate Weather

In the event of bad weather, Atlantic Care Associates will follow the lead of the Chesapeake City Courts. If they are closed for weather, our office will also be closed. Please know that the health and safety of our staff and patients is our priority. ​
